The Enhanced Doctor-Chapter 651 Difficult Position Tumor
After taking a small walk, Liu Banxia was feeling much calmer.
Just as one should eat food mouthful by mouthful, problems should also be tackled bit by bit. Presently, he had inadvertently been pushed into the middle of a storm once again. All he needed to do was work hard at his job. Overthinking random things didn't yield any benefits. Apart from getting himself all worked up, it didn't serve any purpose at all. If his mind was distracted, it might even affect his patient consultations. This included his teaching tasks; he couldn't actually set detailed standards at the moment. He had to evaluate the condition of the interns and residents. Otherwise, any plans he made would be in vain. As of now, there was no final assessment nor announced procedure; he didn't need to panic unnecessarily.
Returning to the main hall on the first floor, he was pretty satisfied with the current situation. At least, the emergency center was performing beyond expectations when it came to critical care services. Even though doctors from the general surgery department took up a large share, there was also collaboration from other departments. Not all emergency patients required general surgery; others might need help from orthopedics, cardiothoracic, or neurosurgery departments.
Thinking about this, he was reminded of the orthopedics department. Wu Mingyu was still holding down the fort alone for now, and Liu Banxia himself would occasionally moonlight there too. He was really hoping others would be assigned to join them soon.
In reality, orthopedics was a large department, with many specialized orthopedic hospitals. With the presence of someone like Director Xu, the number of patients visiting the orthopedics department was also very high.
He decided to set aside other matters and take a look at the operating room schedule. He also needed to help his colleagues shoulder some of the pressure by performing more surgeries.
"What are you up to? You look a bit down." Zhou Li wandered over while he was making his selection.
"Just feeling a bit burdened with my recent responsibilities. Do you have some free time right now?" Liu Banxia asked.
"When is there ever free time? But you could say I have a moment to rest for now. The number of patients is increasing, and there are more matters that need to be handled," Zhou Li responded. "Once our emergency center expands and all departments are fully staffed, could we potentially add more head nurses? Then I wouldn't be so busy, right?"
"What a wonderful thought. But even if we do establish head nurses for the respective departments in the future, have you forgotten what our sign means? Our focus still has to be on trauma emergency services," Liu Banxia said, laughing. "Moreover, to achieve that kind of scale, it wouldn't be possible without at least two to three years. The required hardware and software foundations are massive. Isn't that why we're focusing so hard on teaching and training?"
"However, we shouldn't think too far ahead for now. Let's just patiently endure. We'll get used to it after enduring for so long. I just have to endure three more months, and then I'll be completely liberated."
"That's what you say, but you're dreaming bigger than me. Even if you can truly take a backseat role by then, there will still be a lot for you to handle," Zhou Li responded. "By the way, weren't we supposed to be interviewed for a segment a while back? Why haven't they come over yet? We're looking forward to your next appearance." 𝐟𝐫𝕖𝗲𝘄𝚎𝗯𝕟𝐨𝕧𝐞𝚕.𝕔𝕠𝐦
"They'll probably be here in a couple of days; the hospital is likely waiting for the results of the fecal transplant patients," Liu Banxia said. "This matter is incredible. Even if we've merely run into luck and helped the patients get better, we've truly filled a gap in our hospital's services. That would be a first for Binhai City."
"Also, speaking of this, I wanted to bring something up. If our emergency center really launches a dedicated fecal transplantation program, what's your opinion? Should the nursing staff for it be fixed, or should we stick with the current model?"
"Wouldn't we have to proceed as we do now? The patients have to be admitted to the internal medicine ward, don't they?" Zhou Li remarked. "If we assign dedicated nursing staff again, it would undoubtedly increase the workload of the remaining people. Even if new interns and nurses undergoing standardized training eventually join us, it's hard to say whether that would improve the situation."
"Actually, no matter which hospital it is, there are never enough medical staff. Everyone has to act as a jack-of-all-trades. And even if a dedicated department for fecal transplantation for intestinal diseases was established, there wouldn't necessarily be many patients, right? Three wards should be nearly enough. A transplant every three or four days would mean three transplants are completed in about nine to twelve days. The beds wouldn't be occupied for very long."
"Unless they are all like the patient with Clostridium difficile; that would be a problem. Each caregiving task for such cases is laborious. Otherwise, a nosocomial infection might break out."
Liu Banxia nodded. "I did think about undertaking this project, but it still depends on Wang Chao's thoughts. If he doesn't want to be part of it, we have to find someone else to step up. I'll go persuade Wang Huan in a while. If we have another person to assist Wang Chao, the workload wouldn't be as intense, and the work could also be managed much more effectively."
"Is the main reason for hesitating because fecal transplantation does not have much theoretical data to support it?" Zhou Li asked with a smile.
Liu Banxia gave a truthful nod.
That was indeed the case. Even if you wanted to research this, you might spend a lifetime without making any progress or yielding any results. There were too many factors involved, and the various classifications and combinations of microorganisms could produce too many different effects.
"Actually, I think you all think too much. Even if we don't understand why it works, isn't it enough that it's beneficial?" Zhou Li said. "Just like our current patient, even if he underwent intestinal resection surgery, we don't know what the future will bring. But what if the fecal transplant is successful? It would gift him a normal second half of life."
"Also, currently the initial preparation work is quite extensive, right? If we can get all the preparatory work done in advance, then when a patient comes, we can perform the transplant immediately after their pre-transplant preparations are complete."
"Your thoughts align with mine, so I'm also considering taking on this project." Liu Banxia nodded. "The first time around, we didn't understand the whole situation, so everything was done in a hurry. But with careful preparation, we can establish a standard procedure. Most of the work is done in the preliminary stage, with fine-tuning and the transplantation itself done in the later stage. From a profit perspective, I think it's quite feasible. It's also beneficial to the patients."
"However, it will be a bit tough for the doctors doing it. So, Wang Chao is somewhat hesitant. I still have to figure out how to persuade Wang Huan and Xu Hui."
Zhou Li chuckled. Only Liu Banxia would think of 'coaxing' them like that.
"President Liu," Qi Wentai said, walking over with a CT scan, "look at this patient's condition. Can he still undergo surgery?"
After taking the CT scan, Liu Banxia inserted it into the viewing light immediately. "Wow, this one is very difficult." After examining it, Liu Banxia sighed.
Initially, he thought the patient would be in the late stages of some type of cancer. But the actual condition was different. The patient had a retroperitoneal tumor, which was challenging to manage.
"Yes, I've been studying it for over half an hour," Qi Wentai said. "Initially, I planned to operate around the azygos vein and hemiazygos vein. But there are lots of lymphatic tissues, fatty tissues, and the sympathetic nerve chain in the surrounding area. What's worse, the tumor has pushed the inferior vena cava against the aorta. No matter what I consider, I am not confident about being able to remove the tumor without damaging the aorta. The tumor's positioning is problematic."
Liu Banxia nodded. "That's true. Even vascular clamping wouldn't work. It could easily cause ischemic necrosis to other abdominal organs and the kidneys."
"Can you perform the surgery?" Qi Wentai asked.
"Based on the current line of thinking, I'm unsure," Liu Banxia shook his head. "The location of this tumor is tricky, plus it's rather large. If we were to attempt an autotransplantation of an organ, it would be extremely risky. Have you discussed the situation with the patient?"
Qi Wentai nodded. "Yes, I've spoken with him. He's waiting for us to discuss the surgical plan and see if surgery is an option."
"Tell them not to rush. We'll take our time to study it. We have to find a way; otherwise, letting the tumor grow isn't a good outcome," Liu Banxia said.
Qi Wentai once again nodded and walked over to communicate with the patient.
Liu Banxia turned his attention back to the CT scan, carefully studying it. Many times, this was the kind of helplessness surgeons faced. The decision to operate isn't always straightforward. Sometimes tumors grow too rapidly and lose their surgical value. Other times, as was the present case, the tumor's location complicates matters. The challenge now was how to remove the tumor without damaging other organs or blood vessels. This required finding a better surgical approach to avoid the risks.
However, even after Qi Wentai finished discussing with the patient and came back, Liu Banxia still had not figured out a better approach.
"This case really is challenging," Liu Banxia said. "The time needed to remove the tumor could be extensive. Even if we were to consider the risky approach of temporarily removing the kidney for an autotransplantation, the danger is still very high. Not to mention, the kidney isn't the only organ that needs a blood supply. Vascular clamping is not a viable option. We still need to find a new surgical approach. If we find one, we can deal with this tumor."
"I personally lack the skills to do so. Dr. Shi is in an emergency surgery, so I had no choice but to consult you," Qi Wentai said with a bitter smile.
"I'm somewhat at a loss too. This tumor's location is truly treacherous," Liu Banxia said, staring at the CT scan.
DING!
Task Published: Difficult-to-Approach Tumor
Open-ended task: Respond to Qi Wentai's consultation request and find the best way to remove the tumor. Rewards will be issued based on the diagnostic results.
Qi Wentai sighed inwardly. Liu Banxia was his only hope. If Liu Banxia couldn't come up with a good idea to resolve the current predicament, then this tumor truly couldn't be removed. It wasn't that he hadn't considered asking Zhou Shuwen. While Zhou Shuwen had more experience, Qi Wentai felt that when considering a novel approach, Liu Banxia was more likely to come up with something brilliant. Of course, if Liu Banxia was also stumped in the end, he would have to consult Zhou Shuwen.
