However, according to the different stages of the primary liver cancer course, the selection of blood-invigorating and stasis-resolving medications also varies.
For postoperative patients, due to local vascular and lymphatic obstruction after surgery, blood stasis, and vessel blockage, qi and blood deficiency, it is suitable to choose potent blood-invigorating and stasis-resolving medications;
For some patients undergoing chemotherapy where the genuine qi is deficient, spleen deficiency with dampness, internal dampness combined with stasis, and blood stasis syndrome is further aggravated, it is not suitable to use overwhelming blood-invigorating medications. Instead, facilitate blood circulation and invigorate blood while also supplementing the spleen and kidneys to improve the body's tolerance to chemotherapy;
If the patient is undergoing radiotherapy, where heat pathogens (radiation) are blazing, depleting qi and yin, resulting in a lack of fluids, and sticky blood, aggravating blood stasis syndrome, it is appropriate to use products that cool blood and invigorate blood, to reduce the damage of heat pathogens to normal tissues and enhance the sensitivity of radiotherapy.
In the case of hepatic arterial chemoembolization, it is advisable to coordinate with qi-regulating and blood-invigorating prescriptions. However, if the condition progresses to a stage accompanied by coagulation dysfunction, that is the stage of yin deficiency and blood stasis, early intervention with methods that nourish yin, generate fluids, cool blood, and invigorate blood using a modified Yiguan Decoction can promote liver cell repair and proliferation, maintain normal coagulation status, prevent hemorrhagic events, and extend the patient's survival time as much as possible.
Nevertheless, general liver cancer patients can be categorized into four syndrome types: liver qi stagnation with spleen deficiency, qi stagnation and blood stasis, liver and gallbladder damp-heat, and liver and kidney yin deficiency. The current patient is a typical case of liver qi stagnation with spleen deficiency and internal damp-heat.
If I were to treat, the approach would be to invigorate the spleen and soothe the liver, clear heat, and drain dampness."
